O HEAVENLY Father, I, Thy unworthy servant, presume to appear before Thee this day, to offer unto Thee by the hands of our great High Priest, Thy Son Jesus Christ, the Sacrifice of His most precious Body and Blood. I offer it, O my God, in union with the Sacrifice of the Cross, and with the same intention wherewith He offered it. I offer it to the honour and glory of Thy divine Majesty, in remembrance of the sacred Life and Passion of Thy Only-begotten Son, by the sanctification of the Holy Ghost, the Comforter. I offer it in thanksgiving for the blessings bestowed on Thy whole Church, and for those vouchsafed to me, though most unworthy, [especially ....]; for the pardon, absolution, and remission of all my sins, [especially ....], and of those for whom I am bound to pray; for my own salvation, for the salvation of the living, [especially . . . .], and for the repose of the faithful departed, [especially ....]; and lastly, O Lord God, I offer it for obtaining all those gifts and graces, [especially ....], which Thou knowest to be needful for myself, and others for whom I desire to pray.
